TI-RADS 5 Indeterminate Thyroid Nodules Show High Malignancy Risk Even Without Molecular Alterations

In cytologically indeterminate thyroid nodules (AUS/FN), TI-RADS 5 lesions had 56% malignancy risk. Even without mutations, AUS in TI-RADS 5 had 44% risk vs <10% in TI-RADS 3-4. Combining TI-RADS and Bethesda may refine risk stratification.
- Among 919 indeterminate nodules, malignancy risk rose with TI-RADS: for AUS, 14% (3), 31% (4), 60% (5); for FN, 26%, 44%, 57%.
- Without molecular alterations, ROM was <10% for TI-RADS 3-4, but reached 44% for AUS nodules in TI-RADS 5.
