Targeted biopsy alone yields comparable cancer detection to combined biopsy in PI-RADS 4-5 lesions

For PI-RADS 4-5 lesions on bpMRI, targeted biopsy (TB) alone vs combined TB+systematic biopsy: csPCa detection 67.9% vs 69.7% (P=0.373, Kappa 0.958). PCa detection 79.6% vs 82.2% (P=0.136). TB may be an equivalent alternative.
- Agreement between TB alone and combined biopsy for csPCa was near perfect (Kappa 0.958), with similar detection in PI-RADS 4 (59.2% vs 61.5%) and PI-RADS 5 (82.7% vs 83.7%).
- In the radical prostatectomy subgroup, the rate of upgrading from biopsy-negative or ciPCa to csPCa was 12.3% for TB alone and 9.9% for combined biopsy.
