Cardiac MRI-based SCAN-MRI risk score outperforms clinical scores for heart failure events in diabetes

In 1388 adults with type 2 diabetes, a cardiac MRI-based risk model (SCAN-MRI) achieved an AUC of 0.76 for predicting heart failure hospitalization or cardiovascular death, significantly better than existing clinical risk scores (AUC 0.66 and 0.65, both P<.001).
- Adding cardiac MRI markers to the WATCH-DM risk score improved its AUC from 0.66 to 0.74 (P<.001) and to the TRS-HFDM score from 0.65 to 0.73 (P<.001).
- The SCAN-MRI risk score incorporates sex, coronary artery disease, age, atrial fibrillation, NT-proBNP, and MRI variables, and was externally validated (C index 0.71).
