Primary iliac vein stenting effective for hemodynamically significant nonthrombotic lesions with pelvic pain

Journal of vascular and interventional radiology : JVIR2d ago

Primary iliac vein stenting reduced pain (7.7 to 1.4, p=0.01) and pelvic reflux in 69.4% of women with hemodynamically significant nonthrombotic iliac vein lesions and ovarian vein reflux. For non-significant stenosis, ovarian vein embolization/resection alone was effective.

  • Prospective cohort of 89 women guided a treatment algorithm based on hemodynamic significance of iliac vein stenosis.
  • Primary ovarian vein embolization in patients with significant stenosis did not improve pain or reflux; secondary stenting was successful.
  • In non-significant iliac lesions, ovarian vein embolization/resection alone reduced pain from 6.8 to 1.2 (p=0.01).
