Automated volumetric iRECIST scoring on CT improves interobserver agreement in metastatic renal cell carcinoma

In a retrospective study of 75 metastatic RCC patients, an automated volumetric iRECIST tool on CT improved interobserver agreement from κ=0.34 (95% CI 0.11-0.56) to κ=0.67 (95% CI 0.40-0.89; p=0.04).
- The automated tool achieved a tracking accuracy of 0.98, F1 score of 0.90, and new lesion detection accuracy of 0.85.
- Manual iRECIST scoring had moderate agreement with consensus (κ=0.60 at first follow-up, κ=0.44 at second follow-up).
