Shrinking IPMN Cysts Associated with Greater Risk of Concomitant Pancreatic Cancer

Cyst shrinkage in IPMN paradoxically associated with 4.46-fold higher concomitant PDAC risk (95% CI 1.62-12.30) in a retrospective cohort. No PDAC in growth group; competing IPMN-derived carcinoma. Findings challenge benign interpretation.

  • Retrospective single-center cohort of 1103 patients with branch-duct or mixed-type IPMN followed ≥1 year.
  • Concomitant PDAC occurred in 2.1% of shrinking cysts vs 1.1% stable and 0% growing; growing cysts had 9.7% IPMN-derived carcinoma.
  • Exploratory: only 14 concomitant PDAC events; external validation needed before modifying surveillance.
