MRI-derived heterogeneity score predicts 21-gene recurrence risk in ER+/HER2- breast cancer with external validation

MRI-derived intratumoral heterogeneity score (ITHscore) improved 21-gene recurrence score prediction in ER+/HER2- breast cancer: AUC 0.86 internal, 0.82 external. n=682, retrospective.

• Retrospective, internal training/test (n=452) plus external Duke cohort (n=230); model built with SVM.
• ITHscore significantly higher in high-risk vs low-risk patients (p<0.001).
• External validation AUC 0.82, but high-risk prevalence was low (19%), limiting generalizability assessment.
